Connecticut Community Care Inc. (CCCI) is a not-for-profit that coordinates long-term community-based care for 14,000 individuals across 123 towns in Connecticut. CCCI is dedicated to partnering with individuals, families, supporters and long-term providers so that people of all ages and abilities may remain independent and living at home. SMC documented detailed process flows for all of CCCI’s federal, state, and private programs, including mapping CCCI’s usage of more than 20 technology systems and software programs.
We also documented business and technical requirements for a new care management system and evaluated possible system solutions based on the requirements, the company’s strategic vision and goals, and a five-year total cost of ownership financial projection.
SMC designed and developed an enterprise-wide care management system that supports key care management processes including: assessment and care planning of individuals, program-based workflow and task management, billing rules engine to determine and prepare bills, reporting and business intelligence, system interoperability including exchanging data with external payers and providers, and system/data administration functionality.
The system also includes a data mart to process complex state requirements for managing service authorizations for client care plans. In addition, SMC has provided leadership and project management with process improvement initiatives leading to increased efficiency, effectiveness, and revenue for CCCI, as well as implementing short-term solutions to address the most pressing challenges with CCCI’s current system. Together, we are supporting thousands of individuals of all ages and abilities and helping them achieve their dreams to live how and where they choose.
