Connecticut Association for Community Action (CAFCA) and its ten member community action agencies (CAAs) reduce the conditions of poverty by providing local services to individuals and families and referring them to state and federal programs. SMC designed and built their existing statewide client database, the “DataBridge”, which collects and reports demographic, program, service, and outcome information for individual, unduplicated clients. The DataBridge shows the total picture of client interactions across multiple agencies and programs. Now a funder or legislator or program planner can see the positive impact of investments made in their communities. SMC also designed and implemented the online “Automated Benefits Calculator” (ABC) to help agencies, community health workers, and individuals find appropriate social services programs based on their needs, demographics, and program eligibility criteria.

SMC worked with New Opportunities Inc., a community action agency in Waterbury, CT, to design and develop a custom case management software solution. This system enables case managers to more effectively assess clients’ strengths and weaknesses, refer these individuals to the best services available and manage them through to achievement of planned outcomes.  The system assesses any number of social dimensions of health, such as nutrition, child care, energy assistance, housing, health care access, transportation, employment, and more.  This web-based application streamlines the case management process with automated notifications, referral tracking, and document management.  Case managers are able to effectively and proactively serve their clients while facilitating accurate client tracking and reporting with unduplicated data.    The system produces longitudinal reports of outcomes for each addressed dimension of health for each client along with required federal Results Oriented Management and Accountability (ROMA) reports for the unduplicated client population.

Now SMC and NOI have partnered to offer their solutions to other human services agencies throughout the country.  The commitment is to create a community of users and reserve a portion of subscription fees to enhance the solutions based on the needs of the community.  The vision is to create a rich database of client information that can be used to inform policy and enhance the lives of all individuals and households served.
